情報につきましては入山届・保険加入等のガイド準備に使います。ご記入いただいた内容を第３者に譲渡すことは一切ありません。

マウンテンガイド・コヨーテの主催するガイドツアー及びレッスンに参加するにあたり以下の事項を理解した上で入会します。

1. 自然の中での活動であり予測不能の事態が発生する可能性があります。参加に当たってはこれら危険（自分または他人の行為による財産上、身体上の損害を含む）があることに同意の上、参加いたします。
2. 貸し出された用具等を紛失損壊した場合は弁償していただく場合があります。
3. 参加当日の体調などから、参加不適当な場合にはご参加をお断りさせていただく場合があります。
4. 参加中、体調が悪くなった場合は早めにスタッフに伝えるようにいたします。
5. 持病（特に心筋梗塞・狭心症など）のある方は、事前にスタッフに必ず伝えます。

1. This activity will take place outside in Nature, and unforeseen situations and unpredictable events can occur. I understand and accept the possibility of danger, and of injury and/or damage or loss to my possessions, due to my own actions or those of others.
2. If and when I damage or lose any equipment belonging to Mountain Guide Coyote, I agree to fully compensate Mountain Guide Coyote for any such damage or loss.
3. Mountain Guide Coyote may, at it's own discretion, deny participation to anyone who, in the opinion of Mountain Guide Coyote, is not in a good enough physical condition on the day in question, to participate in the planned tour and/or lesson.
4. I agree to immediately, and without fail, to inform Mountain Guide Coyote staff if, due to injury or any other cause, my condition deteriorates during the tour and/or lesson.
5. I will always inform staff in advance if I have any medical conditions. I will always inform them of any medication I am carrying.
